Join Optum's Senior Community Care team and provide meaningful, longitudinal care to medically complex patients in community based residential settings throughout Multnomah and Clackamas Counties.
As the primary Advanced Practice Clinician for your patient panel, you make the clinical decisions while being supported by a fully integrated interdisciplinary team. Our value-based model focuses on quality of care, not patient volume or productivity quotas.
This is a full-time, field-based position providing longitudinal care to patients in a variety of community-based residential settings, including assisted living, independent living, adult family/foster homes, senior living apartments, and private homes.

Make an Impact with Complex Patient Care
As part of Senior Community Care, you'll care for some of Optum's highest-risk Medicare members, including patients with multiple co-morbidities, advanced disease states and complex treatment needs.
You'll address the whole patient, including medical, behavioral, functional and social needs, while partnering with caregivers, facility teams and other healthcare professionals to improve outcomes and quality of life.
Primary Responsibilities
- Conduct comprehensive patient assessments
- Manage acute and chronic conditions, including testing, diagnostics and interventions
- Develop and manage complex treatment plans
- Lead advanced illness, goals-of-care and advance care planning conversations
- Collaborate with the interdisciplinary care team, facilities and caregivers
- Provide patient and caregiver education
- Ensure accurate and complete clinical documentation

Required Qualifications:
- For Nurse Practitioners:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- Active and unrestricted Nurse Practitioner license in the state which you reside, or ability to obtain by start date
- Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:
- Family Nurse Practitioner
- Adult Nurse Practitioner
- Gerontology Nurse Practitioner
- Adult-Gerontology Acute Care Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- For Physician Assistants:
- Graduate of an accredited Physician Assistant degree program
- Active and unrestricted Physician Assistant license in the state which you reside, or ability to obtain by start date
- Board certified by the National Commission on Certification of Physician Assistants (NCCPA)
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice) (SCC04)
- Ability to gain a collaborative practice agreement, if applicable in your state
- Ability to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, remain in a stationary position and position self to perform physical assessment
- Driver's license and access to reliable transportation
Preferred Qualifications:
- 1+ years of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Understanding of Advanced Illness and end of life discussions
- Proficient computer skills including the ability to document medical information with written and electronic medical records
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $91,700 to $163,700 annually based on full-time employment. We comply with all minimum wage laws as applicable.
