As the CORE Manager, you will oversee the home health referral process by managing a team that supports all administrative and operational activities to ensure complete, timely, and accurate referrals are processed and transitioned to the agency for evaluation and care. This role involves meeting standard KPIs, collaborating with other departments for continuous improvement, and acting as a liaison between the business development team and the clinical/operational team.

Primary Responsibilities:

  • Ensures the CORE team is adequately staffed according to the staffing model
  • Responsible for selecting team members, orientation and training, and timely evaluations
  • Oversees day-to-day workflow and sets priorities for staff
  • Works with leadership to develop training materials and ensure updates as needed
  • Monitors referrals to the agency and reviews routine reports to ensure goals are met, escalating issues where appropriate
  • Develops subject matter expertise in multiple technology systems ( Forcura, Homecare Homebase, and others) and makes recommendations for system/process enhancements
  • Serves as a liaison between business development/referral sources and the agency to support timely, complete, and accurate referrals, positioning those referrals for timely decision-making and admissions
  • Understands and supports admission criteria, both clinical and socio-economic, to facilitate timely decision-making and admissions

Required Qualifications:

  • Proficient in Microsoft Office applications (Word, Excel, etc.)  
  • Demonstrated ability to manage a team and keep team accountable to specified goals and metrics
  • Demonstrated critical thinking and problem-solving abilities
  • Demonstrated ability to develop training materials and oversee team workflow

Preferred Qualifications:

  • Bachelor's Degree in a healthcare field
  • 2+ years of managerial experience
  • Experience in home health or a similar healthcare setting and familiarity with referral processes
  • Clinical background (e.g., RN, PT, OT, ST)  
  • Proven excellent interpersonal, written, and oral communication skills
  • Proven solid customer relations skills and ability to work in a team

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $49,700 to $88,800 annually based on full-time employment. We comply with all minimum wage laws as applicable.
