Senior Community Care ('SCC') is designed to support senior living communities to provide care for their residents. Our approach is designed to reduce friction for members by addressing their physical, mental and social needs according to their personal care preferences and the location they desire for care.
SCC currently serves over 900 senior living communities and 5,000+ IE-SNP members. One of the unique features of our model is the introduction of a care navigator to each member. The care navigator acts as the "quarterback" of the care team, with a primary focus on connecting members to doctors, coordinating care needs - including member benefits - and ensuring members receive their annual comprehensive visit. They also link members with local resources such as transportation, home health and food assistance, identify Stars gaps and help facilitate closure, and support UHC sales agents.
