Care At Home ('CAH') is an integrated care delivery provider, managed by a comprehensive interdisciplinary team focused on delivering high-touch, community-based support directly in the homes of our most fragile, medically complex, and costly populations. Our approach is designed to reduce friction for members by addressing their physical, mental, and social needs according to their personal care preferences and the location they desire for care.
We currently serve over one million members across more than 17 markets. One of the unique features of our model is the introduction of a care navigator to each member. The care navigator acts as the ""quarterback"" of the care team, with a primary focus on connecting members to doctors, coordinating care needs-including member benefits-and ensuring members receive their annual comprehensive visit. They also link members with local resources such as transportation, home health, and food assistance, identify Stars gaps and help facilitate closure, and support UHC sales agents.
